How to avoid losing muscle on Ozempic (without living at the gym)

You stepped on the scale and it's down 25 pounds. Great news — until you notice the stairs feel harder and your grip is weaker. Here's what's happening: rapid weight loss on a GLP-1 doesn't only burn fat. A meaningful chunk of what you lose can be muscle, and the scale will never tell you which.

How much muscle are we talking about?

Across recent trials and reviews, roughly a quarter to 40% of GLP-1 weight loss comes from lean mass. In the STEP-1 semaglutide trial, lean mass dropped about 9.7% while fat dropped 19.3%. Muscle is your metabolism, your strength, and — after you stop the drug — your best defense against regain.

The scale says success. Your legs on the third flight of stairs disagree. That gap is muscle.

The good news: it's largely in your control

Muscle loss during weight loss is driven more by your habits than by the medication (American Council on Exercise, 2025). Programs that pair a GLP-1 with protein and resistance training see far less muscle loss for the same fat loss. Two levers do almost all the work:

Lever 1 — Hit a real protein floor

Lever 2 — Lift something twice a week

You don't need a gym. Two or three short sessions of squats, presses, rows, and carries — bodyweight or bands at home — signals your body to keep the muscle. Consistency beats intensity here.
